Lead Excursion Teacher Declaration

I understand by ticking the checkbox below I am declaring:

• I am the Lead Excursion Teacher in charge of a school excursion.
• I have read relevant Child Safety, Public Liability and Risk Assessments.
• I will read the School Excursion COVID-19 Guidelines.
• I will communicate as necessary with other Teachers.
• I understand no one should participate in an excursion if they are displaying symptoms of COVID-19.

Please be aware, if you do not submit this declaration your school may have Trips/Bookings cancelled.
